    Advanced Illness Management (AIM)

    At Sutter VNA & Hospice, we’ve seen how the right support can help patients with advanced chronic illnesses experience improved quality of life. When given skilled, respectful coaching and care, patients better manage their health.

    That’s why we created Advanced Illness Management (AIM), a program that partners with your doctors to provide you the nursing and other support you need to address your illness and live your life.

    Care is Customized to Meet Your Needs

    AIM is for patients with advanced illnesses such as neurologic disease, advanced cancer, chronic lung disease and heart failure.

    AIM provides care management services with emphasis on palliative care for patients who may still be receiving active treatment, and are not ready to receive hospice services.

    With every AIM patient, we strive to accomplish these overall objectives.

    Compassionate Professionals Provide Care & Comfort

    The AIM program is staffed by exceptionally committed, caring professionals who have the experience, knowledge and training to help you manage your complex illness.

    Your AIM team will include a nurse and social worker. AIM’s medical director is available to consult with the AIM team and your own physician about your care.
